The Outreach Community Health Worker (CHW) provides preventative health related education through outreach to marginalized community members. In this role the Community Health Worker builds strong relationships and trust with our community and existing social services programs. They will identify barriers in accessing services and will refer patients to various health and social services programs to address those services. The Outreach CHW will use data collection tools to complete reports and summaries as needed. They will facilitate communication between patients/families and the healthcare team to ensure patient satisfaction and quality of care.

This position works both independently and collaboratively as part of the medical practice team. Key services include (but not limited to) problem-solving, access to insurance, access to medications, referrals to community resources, education, and leading educational groups.

Minimum Qualifications

  • Bilingual- proficient in reading, speaking, and writing in English and Spanish.

  • Minimum of 3 years of experience in a health related or social service-related field.

  • Bachelor’s in public health preferred or 3 years of direct experience in a similar role.

  • Experience working with low income and multi-ethnic populations.

  • Experience with training and presentations in education or healthcare.

  • Demonstrated ability of positive interpersonal and communications skills with colleagues, patients, families, providers, and the community.

  • Familiar with medical terminology and the healthcare system including the Oregon Health Plan and Washington Apple Health.

  • Knowledge of local community resources.

  • Strong written and oral communication skills.

  • Experience with using technology tools (i.e. email, spreadsheets, Microsoft Office, voicemail, internet etc).

  • Ability to travel within the gorge is required including a valid drivers license.

  • Ability to work independently, work with multiple projects and maintain a high degree of professionalism and confidentiality.

  • Commitment to providing culturally and linguistically appropriate care coordination with client and family centered approach.

  • Knowledge of a compliance with HIPPA regulations.

Preferred Qualifications

  • Oregon State Community Health Worker certification

  • OHP application assister training
