Title: Charge Licensed Practical Nurse (LPN) -Assisted Living

Full-time Position

Shift: 6pm-6am

Responsibilities Include:

• LPN will provide direct nursing care to residents and supervise the day-to-day nursing activities performed by CNA's.
• Meeting with assigned nursing personnel to plan shifts’ services and activities.
• Overseeing the activity of assigned nursing assistants.
• Ensuring residents receive services in accordance with their care plan.
• Reporting problem areas to the RN supervisor or administrator.
• Be active in the department, assisting and leading the Wellness team and other departments and ensuring quick, reliable, and person-centered care is provided.
• Documenting resident assessments and care by ensuring that internal documents are completed accurately and efficiently.
• Provides compassionate care to residents by ensuring each interaction with the resident is professional, caring and ends with the resident feeling safe and cared.

Qualifications:

• Must be a Licensed Practical Nurse in good standing.
• Assisted living experience preferred.
• Thorough working knowledge of current standards and regulations.
