ModRN Health's Virtual & Onsite Primary Care Nurse has the responsibility and accountability for coordinating the overall health plan for ModRN Health's members. The Virtual/Onsite Primary Care Nurse works with ModRN Health Physicians and are the health care managers of members' health following the proven nursing process. The Virtual/Onsite Primary Care Nurse performs a thorough initial holistic assessment of the member at the member's place of employment (when applicable) and develops a collaborative plan of care with the member. Virtual Primary/Onsite Care Nurse develops a long-term relationship with members to understand personal health goals, measuring progress towards outcomes, members' health care providers and services to optimize their health care journey. ModRN's Health's Virtual/Onsite Primary Care Nurse are role models in the nursing profession, maintaining the highest level of professionalism, exceptional member courtesy, and confidentially at all times. Routinely works with other team members to provide quality member service. Adheres to HIPAA guidelines and ModRN's Health's privacy policies. ModRN Health's Virtual/Onsite Primary Care Nurse are registered nurses that assist our members in understanding their overall personalized care plan, care and treatment options in order to facilitate informed decision-making. The Virtual/Onsite Primary Care Nurse helps a member navigate the complexities of the system including treatment options, care transitions, treatment compliance, provider collaboration and communication, including identification of other community resources available.

Essential Functions: Establishes therapeutic relationships with the assigned member population. Collaborates with members to develop and prioritize a personalized, continuous plan of care that includes physical, emotional, social, cultural and spiritual aspects. Personalized, continuous care plans are created where detail tasks are actionable and measurable activities for the member. Triage health issues (acute care), routine care, health reviews, follow-ups and consults are conducted for numerous points of care such as pre/post appointment preparation and review, medication reconciliation, member education, questions regarding tests and procedures, general triage and recommendations.

Other duties as assigned. Required Education and Experience: Active, unrestricted Maine Registered Nursing license in good standing (ADN and BSN). Ability to obtain a Compact Nursing license. Preferred 5+ years recent Hospital/Clinical experience; preferably ED, Med-Surg, Cardiac or Primary Care. Care management, medical case management or patient advocacy experience a plus. Experience with nurse leadership roles a strong plus (Charge Nurse, Director of Nursing, time served on hospital or clinic boards, peer to peer teaching, nurse preceptor, floor manager, adjunct professor, program development).

Job Type: Full-time

Pay: $70,000.00 - $75,000.00 per year
