• Function autonomously to virtually care for patients with complex, acute, critical and chronic health conditions.
• Work individually and in interdisciplinary teams of clinicians and business professionals
• Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
• Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams. Ensure patient information, visit history, lab/pharmacy/imaging follow-up, phone calls, and consults are accurately documented in the electronic medical record
• Talk to patients remotely about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care.
• Assumes responsibility for progress notes on patient encounters. Maintains accurate and well documented charts.
• Works with nursing staff to ensure optimal patient care and adherence to compliance program policies and procedures. Serve as a resource person for the nursing and medical
  assistant staff
• Demonstrates knowledge of action, dosage and side effects of medication allowed by NYS
  law. Prescribe medications including the frequency and dosage
• Ability to effectively collaborate and build relationships with patients, families and all care providers
• Strong computer skills with knowledge and experience with Electronic Medical Records
• Demonstrates a professional, courteous, and respectful attitude, Excellent technical and patient care skills.
• Models a customer service focus. Respects patient confidentiality, privacy and dignity.

What You'll Need:

• Masters from an accredited Nurse Practitioner Program
• Current NYS Nurse Practitioner certification.
  
• Fluency in Spanish
• Prior experience with telehealth/telemedicine or remote patient monitoring preferred
• High level of clinical proficiency and excellent decision-making skills.
• Ability to work independently and as an effective member of a healthcare team.
• Excellent oral and written communication skills.
• Demonstrated problem-solving and workflow management
• Proven organizational skills and ability to prioritize effectively.
