POSITION: Targeted Case Manager

LOCATION: Elizabethtown, KY

QUALIFICATIONS:

• bachelor’s degree in human services (or a related field)
• targeted case management (TCM) certification
• completion of recertification requirements (KBHDID) every three years
• experience working as a TCM required.

RESPONSIBILITIES AND TRAINING:

• coordinate services for patients with severe emotional disabilities, mental illnesses, and substance use disorders
• perform patient assessments to determine strengths and needs (medical, social, functional, educational)
• perform periodic reassessments
• arrange for transportation and delivery of services
• facilitate referrals
• regularly communicate with patients about their health, well-being, and appointments
• provide documentation (care plans, forms, reports, narratives)
• participate in monthly treatment team with Intensive Health (a Stepworks Company)
• other duties as assigned
• be prepared for crisis intervention, first aid, CPR (training provided)
