Job description

This is a combination of direct service and administrative position, requiring general knowledge of developmental disabilities and co-occurring disorders and specific knowledge and skill in rehabilitation with persons with disabilities. This position involves the provision of day program and vocational services and coordination of overall services for members, and supervision of assigned staff. The Pathways Site Supervisor is expected, through training and experience, to exercise independent judgment with the supervision of the Program Director.

Responsibilities:

  • Supervise assigned staff on the team.
  • Provide smooth day to day operation of assigned areas.
  • Provide orientation for clients assigned to team.
  • Initiate communication with supervisor regarding concerns, ideas, input, suggestions or problems.
  • Develop monthly schedule for staff coverage of assigned members and develop daily coverage plans as needed. Assume bottom-line responsibility for staff coverage of assigned area.
  • Provide and/or coordinate the following services with assigned clients: assessment, planning, linking, monitoring, advocacy and care, both internal to program and through external social service agencies.
  • Assist assigned clients with medication administration and money management according to policy.
  • Provide transportation for members.
  • Implement crisis intervention actions as appropriate, and provide training/direction to staff regarding crisis response.
  • Instruct, schedule, and facilitate (do side by side with client) activities, groups, and assessing the needs of individuals. Coordinate schedules and activity among staff working together with assigned members.
  • Provide oversight of staff interventions, including role modeling and teaching of staff.
  • Promote and insure the health, safety and security, organization, and an atmosphere of the Pathways Program space and office areas, including oversight of inspection preparations and communication as needed.
  • Promote effective relationships among staff, clients, families and service providers, and educate them where appropriate regarding disabilities and organization’s services.
  • In conjunction with manager, participate in developing and implementing individual plans of assigned clients including planning for and/or providing coverage for IRP meetings.
  • Coordinate services to members through effective interface with the various program components.
  • Train and hold accountable all staff on the maintenance of current, accurate and complete documentation related to all service functions in assigned area, including daily progress notes, service tickets, monthly progress notes, incident reports, ABC charts, and individual member report cards.
  • Assist with interviewing and making recommendations for hiring new staff, assist with background checks as needed. Assist with orientation, training, ongoing supervision and evaluation of staff on team. Assist staff with plans to enhance professional growth, including external and internal training and education. Ensure the completion of all trainings for new staff.
  • Participate in the development of the Quality Assurance Plan, and support its activities and goals.
  • Collaborate with Program Director and participate in meetings as assigned.
  • Support Program Director and assigned staff by being willing to perform all duties.
  • Participate in Pathways Leadership to oversee all day program DDA licensing requirements including compliance with COMAR, CARF, Incident Reporting, Quality Assurance, Training and Medication Administration.
  • Ensure staff attend and complete all Sheppard Pratt and DDA required training. Ensure the schedule is developed to ensure that staff are able to attend and serve as back up if needed. Document when training is complete and submit to Human Resources for tracking on the e-Learning system.
  • Make arrangements as needed and as requested for different activities (i.e. reserving parks or picnics).
  • Acquire needed program supplies as requested.
  • Propose to Program Director for external trainers to come to train Pathways staff as requested. Assist in planning for meal breaks and a meeting space as needed for such events.
  • Assist with visitors for Pathways members and staff as needed and requested.
  • Assist in the process of obtaining funding for assistive technology/equipment by informing Program Supervisor of these needed.
  • Perform other duties as assigned.

Requirements:

  • Possession of a high school diploma or equivalent is required, along with two-years of relevant work experience. Associates or bachelor’s degree in related field preferred.
  • Positively referenced, preferably relevant, work, volunteer or student experience.
  • Valid driver’s license and safe driving record.
  • Possession or use of a four-passenger car to be used to transport clients and a copy of a valid certificate of automobile insurance.
  • Must be covered under an active auto insurance policy and a valid copy of auto insurance is required prior to hire.
  • Must obtain and maintain a current DOT certification.
  • Ability to perform individual assessment, planning and implementation in both structured and unstructured settings.
  • Ability to prioritize member needs and manage time to complete assignments daily.
  • Ability to develop and maintain daily structure in day program activities.
  • Ability to assess day programming environments for health, safety, security, organization and comfortable atmosphere.
