Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human serviceAbout the Role: The School Based Case Manager promotes pro-social behaviors and social skills to students within a school-based setting. Provides support to the student to help them achieve academic success. Assist students and families in obtaining recommended service coordination and community support.Qualifications: Bachelor's degree in Social Work or CounselingPrevious experience assisting with service coordination and supports of students and families.Proficiency with computers, iPads and other related technologies and experience entering data into electronic medical records (EMR) program.Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.*Successful completion of a criminal history background check after hire.What's in it for YOU: Full Benefit PackageCompetitive Salary: $20-$23/hourCareer Training and DevelopmentTuition DiscountsCCN is an Equal Opportunity Employer
