Sunny Place Foster Homes has 3 Adult Foster Care Homes. We are looking for a RN that to assist with Health Services Coordination of the program recipients.

Schedule: 20-24 hours per week. 3 days per week. Preference M-W-F.

Duties include but not limited to:

Medication review and set up upon admission

Staff training on medication administration and related policies and procedures

Communication of physical or health changes to the support team

Assistance and coordination of medical, dental and health appointments

Obtaining and reviewing new medication and treatment orders

Creating, reviewing and oversight of MAR's and TAR's

Reviewing medication errors and providing follow up and training as needed

Quarterly reviews and reports of medication created and provided to the support team

Ability to provide phone triage or guidance to staff

Knowledge of 245D statues as they pertain to health service coordination or the ability to read and understand statutes and create and implement policies and procedures as they pertain to the statutes

Job Type: Part-time

Pay: $35.00 - $40.00 per hour
