Hospice Services of Lake County a positive and supportive work environment, is seeking a full time RN Case Manager who will make a meaningful impact in the lives of our palliative care patients. The successful candidate will collaborate with team members to manage symptoms, provide education and assist in navigating health care systems.

Specialized training, coaching and support is provided as needed for clinical team members.

The Registered Nurse Case Manager plans, organizes, and coordinates palliative care under physician’s orders other Interdisciplinary team members. Care and family support is provided in the patient’s place of residence whether it is a private home, a residential care facility, or a skilled nursing facility.

Work hours are Monday-Friday pm from 8:00 am- 4:30 with occasional evening on call coverage requested.

Current California RN license and a minimum of three years of experience preferred. Hospice experience is a plus. Valid driver’s license, reliable transportation, drug screening and background check will be required.
