We are seeking an Assistant Program Director to supervise management of 2 four person Community Residential Service Programs, with initial predominant duties focused on one medical needs program in Plymouth, MN. Position will grow into duties as assigned at additional program, with ongoing growth opportunities available. Job Summary

The Assistant Program Director will support the Program Director with providing leadership in the supervision of Program Managers and Direct Service Professionals (DSP/s) in daily operations of 24-hour Community Residential homes and unit-based services. The Program Director facilitates and assures licensing compliance and coordination of services with County and State agencies, manages financial operations in each home, assures staffing ratios, and assures the person-centered health, safety, well-being and life goals of each person served. Open, fair and person-centered communication with each person served, their IDT members, and employees is essential to this position.

Summary of Responsibilities Report to supervisor in a timely manner all observations regarding consumer health and safety, noncompliance of licensing requirements, employee issues, and home maintenance.

  • Assure compliance, monitoring, and follow up of health and safety and licensing issues.
  • Work with Quality Administrator to complete all consumer paperwork for IDT meetings as required by 245D.
  • Attend all consumer IDT meetings and assure follow up documentation and communication are completed.
  • Assure that all PM’s have the needed supplies and resources to do their jobs.
  • Provide orientation training, mentoring, and on-going training to PM’s as assigned.
  • Respond to emergencies (i.e. consumer safety, hospitalizations, serious injury, staffing issues, and fire) when needed and assure documentation and follow up in regard to incidents are complete.
  • Ensure that PM’s have all staffing secured for shifts; assuring staffing ratio in each home.
  • Respond to and provide on-call staffing needs outside of your regularly scheduled hours.
  • Routinely communicates with families, case managers, day placements, medical personnel, and psychologists and provides follow up and documentation in regard to all requests and concerns.
  • Communicate and follow up with any family and case manager concerns/complaints and communicate all concerns/ complaints to supervisor.
  • Respond and follow up with all Vulnerable Adult reports, EMUR reports, Incident reports, and med errors.
  • Review all nurse consultations and assure follow up is completed.
  • Monitor medication records, medication documentation, and health progress notes on Therap.
  • Plan and facilitate PM and/or APM meetings individually and quarterly or as needed with the full group.
  • Communicate all issues, concerns, and follow up to the Executive Director in a timely manner.
  • Duties requested by Executive Director not listed above; as assigned.

  • Qualifications:
  • Four-year degree
  • One year of supervisory experience
  • One-year minimum experience working in and/or supervising a group home setting.
  • Valid Driver’s License and pass vehicle insurance eligibility requirements
  • Qualified background check through the Bureau of Criminal Apprehension
  • Clear demonstration of written and verbal communication skills
  • Ability to take initiative; problem solve; make independent decisions; participate actively in meetings.

This position reports to the Program Director.
