Job Description

In conjunction with the Clinical Pharmacy Manager, develops, coordinates, and monitors the pharmaceutical care services provided by the pharmacy staff. The Cardiology Clinical Pharmacy Specialist will focus on disease prevention and treatment, including evidence-based medication use and related care that improve both short-and long-term outcomes for patients. Will be responsible for reviewing, analyzing and monitoring multifaced clinical information to make reasoned decisions for patients with multiple comorbidities and highly complex medication regimens.

In conjuction with the Clinical Pharmacy Manager, develops, coordinates, and monitors the pharmaceutical care services provided by the pharmacy staff. Primary focus is on all functions relating to the provision of the clinical pharmacy services. Under direction of the Clinical Manager, develops and maintains the hospital drug formularies with a focus on standardization of the formulary. Assists in developing, coordinating, leading and monitoring all pharmacy department quality assurance programs and education programs relating to medication therapies for pharmacy, medical, and hospital staff. Fulfills the duties of a pharmacist as necessary with a minimum of 40 hours per week, but hours may be long and irregular; includes evenings, weekends, and holidays as necessary. Assists in the planning of pharmacy services and programs to include the budgeting process. Under the direction of Pharmacy Management, promotes pharmaceutical care by: (1) implementing programs to ensure safe and cost effective use of drugs by conducting drug utilization review studies; (2) providing clinical consultative services and drug information to healthcare providers and precepting students; (3) assessing/reviewing the clinical activities of the staff/clinical pharmacists and students; (4) providing oversight to the CMC “formulary management” functions which are under the aeqis of the Pharmacy & Therapeutics Committee; and, (5) collecting and assessing documentation of clinical service effectiveness to assist in developing new or expanded clinical service programs. This position relies on experience and judgment to plan and accomplish goals with a certain degree of creativity and latitude required. Bachelors degree, Pharm D preferred. Must be a registered pharmacist. ASHP accredited residency (PGY-1), current BCPS certification, or equivalent direct clinical experience (with BCPS eligibility) required. ASHP accredited residency (PGY-2) in cardiology preferred with current board certification in cardiology (BCCP).
