- 403(b) Retirement Plan with generous company contribution = 8.5% match after one year
- 11 Paid holidays, vacation ( 10+ Full Time), and sick leave accrual up to 90 days
Purpose
Develops and maintains strong relationships with internal and external partners to ensure delivery of quality care and services to members. This position is full time Monday through Friday from 8am to 4pm with flexibility to work outside of these hours/days as needed.
Essential Functions
- Builds relationships with members and their families through meetings, scheduled home visits, participation in programs and activities, and other correspondence.
- Ensures that services and care are provided as outlined in the member agreement and member’s individual service plan.
- Develops and maintains strong relationships with internal and external partners to manage and enhance the members’ experience.
- Works with Member Services Team to coordinate and schedule requested care and services, address concerns and collaborates to find workable solutions.
- Conducts in-person visits with members on a monthly basis (or as requested by the member or more frequently as needed) to identify:
- issues/trends and analyze root causes to determine areas of improvement or if further action is needed for health safety and welfare of the member.
- any additional support that the member may benefit from including possible health care services or referral to outside resources.
- anticipated future needs of the member, based on noted physical/cognitive/social changes.
- Works with Member Services Team to plan and coordinate events/outings at 15 Craigside and Arcadia as well as points of interest in the external community.
- Directs initiatives to ensure continuous development of the program including developing partnerships with outside organizations and service providers to expand available resources for members.
- Enters and maintains member information within electronic systems to analyze service trends, wellness initiatives and improvement opportunities.
- Reviews and processes monthly billing statements for members.
- Works closely with Sales & Marketing team and HCBS COO to increase census through Member Referral Program and identify trends in membership.
- Assists with sales presentations and informational sessions as needed.
- Directs operations and program initiatives to meet budget and other financial goals.
- Maintains on-call after-hours status which may require assisting a member in an emergency situation or coordinating scheduled care of service being provided after normal business hours.
- Other duties as assigned by the Arcadia Family of Companies.
Supervisory Responsibilities
Directly supervises the Member Services Coordinator. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding, counseling and disciplining employees; addressing complaints and resolving problems.

Education and/ or Experience
Bachelor’s degree from a four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Experience in Home Care coordination a plus.

Physical Demands
While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to stand, walk, sit, and taste or smell. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl; push and pull. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Work Environment
While performing their essential functions, employees are occasionally exposed to the following conditions:
- Air conditioned office environment;
- Indoors without air conditioning;
- Outdoors—all types of weather; and
- Hazardous materials.
