Montefiore Einstein Advanced Care is a premier ambulatory diagnostic clinic proudly serving Westchester County. We currently have an exciting opportunity available as a Licensed Practical Nurse that is based out of our Elmsford, NY location.

Job Summary:

The LPN will work with clinical and clerical support staff to deliver excellent patient care and promote patient satisfaction across multiple practices. The LPN will practice under the direction of the appropriate clinical staff and cross cover across multiple practices.

Specific Duties and Responsibilities:

- Monitoring of patient's vital signs, patient intake and output, and recording patient history

- Reports data relating to patient's health status to appropriate clinical staff

- Identifies patient goals, communicates and collaborates with clinical team to support patient care plan

- Administration of medication, injections and functions in scope of license

- Cleaning of equipment in patient care areas

- Maintain patient care rooms to ensure they are kept clean and supplies are well stocked.

- Maintain records of patient activities and observations

- Submit records to the appropriate clinical staff regarding patient's condition

- Assist with scheduling patients, authorizations and referrals to support office staff as needed

- Cross cover multiple practices

Education & Experience:

- Associates Degree

- 2 years in clinical setting

Licenses, Registrations and/or Professional Certifications:

- New York State Licensed Practical Nurse license and registration

- National Council Licensure Examination for Practical Nurses (NCLEX-PN) or another license examination acceptable to NYSED
