Title: Home Health RN Clinical Manager

Reports to: Director of Clinical Operations

Pay: Up to $55/hour DOE

Schedule: Monday-Friday 8am-5pm

We are looking for an enthusiastic and experienced RN Clinical Manager to join our team. We would love to put your clinical & management skills to work. You must be a licensed RN in the State of Oregon to apply. Non-licensed applicants will not be considered.

Position Summary:

The Clinical Manager is responsible for the oversight of a clinical team, including the planning, controlling, organizing, and leading of the team in the provision of effective and efficient clinical services. The Clinical Manager supervises, supports, coaches and educates staff at the point of service in the field and in the office environment.

Essential Responsibilities and Duties:

1. Clinical and Office Productivity Goals

o Goal is that Clinical Managers work with office staff and clinicians to achieve agency productivity standards.

o Tools to help in this process

  • DAR review.
  • Semi monthly Productivity Report based on hours worked.
  • SHP Scorecards

1. Employee Goals

o Establish “one minute goals” for each direct report. These goals would be reviewed quarterly with each employee

o Timely and thorough staff reviews. Reviews should be approved by Director of Operations prior to review.

o Timely and effective conflict resolution with clinical and office staff.

o Clinical Managers to ensure that no time off is approved that creates coverage problems. All contract staff should fall within the budgeted amounts.

3. Clinical Manager Field Supervision Goal (2+ visits per employee per year)

o Goal is for Clinical Managers to spend at least 20% of week in direct supervision of clinical staff educating and supervising time management, clinical practices and OASIS assessment standards using the OASIS walk tool from Fazzi.

o Success in this area will be measured by

  • Time in field recorded accurately, “Visit” time being the greatest, representing the visit & point of care documentation
  • Implementation and ongoing assessment of the OASIS competency process
  • Clinicians meeting productivity expectations

4. Quality Outcomes and Clinical Practice Standards Delivery Goal

o It is important that the Clinical Manager is actively identifying policies/procedures that need improvement and is working to update and educate staff on the new more effective processes.

o In conjunction with the PI/ED director develop and implement a process to hold clinicians accountable to meeting agency standards such as OASIS competency.

5. Weekly Report to Director of Clinical Operations

o Clinical Manager to provide written/oral report on following:

  • Employee Concerns (clinician status, performance, discipline, reviews, overtime, etc)
  • Productivity/Employee Case Loads
  • Clinical Concerns (IDT, process/procedure changes)
  • Quality Outcomes and Clinical practice standards delivery (field supervision)
  • KPI update
  • Other concerns

Qualifications/Special Skills:

  • Familiar with home care and its implementation
  • Skilled in the nursing care process
  • Skilled in the techniques of therapeutic communication
  • Able to coordinate the resources of the Agency
  • Able to facilitate cooperation and communication among team members
  • Demonstrates leadership abilities

Experience and Professional Requirements:

  • Oregon RN licensure with current registration to practice
  • Previous experience in home health or hospice strongly preferred but not required
  • Former management of a team of at least 20+ staff
  • Must have previous experience managing other RNs and clinicians
