Job Summary:

We are seeking a highly motivated and skilled Registered Nurse (RN) Case Manager to join our home health team. The RN Case Manager will be responsible for managing patient care and coordinating services for homebound patients. The successful candidate will have excellent clinical skills, be organized and detail-oriented, and possess strong communication and problem-solving skills.

Key Responsibilities:

• Perform comprehensive assessments of patients and develop individualized care plans in collaboration with the patient, their family, and the interdisciplinary team
• Coordinate and oversee the delivery of medical, nursing, rehabilitation, and support services to patients in their homes
• Provide skilled nursing care as necessary, including wound care, medication administration, and symptom management
• Communicate regularly with patients, families, physicians, and other healthcare providers to ensure the provision of high-quality, patient-centered care
• Monitor patient progress and adjust care plans as necessary
• Document all patient care activities in compliance with regulatory and agency requirements
• Educate patients and their families on disease management, medication management, and self-care techniques
• Participate in quality improvement activities and interdisciplinary team meetings

Qualifications:

• Active RN license in the state of practice
• Minimum of one-year clinical experience as an RN, preferably in home health or a related field
• Excellent clinical assessment and critical thinking skills
• Strong communication and interpersonal skills
• Ability to work independently and as part of an interdisciplinary team
• Proficient in computer skills, including electronic medical records
• Current CPR certification
• Valid driver's license and reliable transportation

Working Conditions:

This position involves regular travel to patient homes and requires the ability to lift up to 50 pounds. The RN Case Manager will work primarily in a home healthcare environment, with some office work required. The position may require some evening and weekend work to meet patient needs. Pay: $73,000.00 - $76,000.00 per year
