Job Summary:
To collect, abstract, submit, validate and analyze reliable data through high quality data compilation, documentation and entry into various registries databases.
Education & Experience
Associates degree in health informatics or information management from an accredited program by the commission on accreditation for informatics or health information management education (CAHIIM) and two years related experience relative to data abstraction, data entry, data analysis, project management.

CERTIFICATIONS, LICENSES OR REGISTRATION REQUIRED: NA
Knowledge, Skills & Abilities
Knowledge of coding to include procedures. Knowledge of electronic medical record. Knowledge of medical terminology and patient charts; skill in the use of personal computers and related software applications; ability to read and interpret medical terminology; verbal and written communication skills; interpersonal skills.

RESPONSIBILITIES
  • Maintains and manages registries for department as directed by accrediting bodies, regulations, reimbursement requirements, an quality improvement.
  • Abstracts medical information and codes from EHR and other sources as needed into appropriate registry or databases in timely manner with attention to the abstraction deadlines set foth by the various registries and/or accreditation audits.
  • Follows accreditation, registry, and departmental policies and procedures in data review and entry.
  • Notes deficiencies in medical records documentation needed for abstraction; Monitors reports for missing documentation, requesting patient records as needed.
  • Collaborates with personnel regarding medical record issues and missing or incorrect data, or inconsistencies; follows up with data deficiencies to ensure completion.
  • Maintains knowledge of medical terminology, documentation trends, and high proficiency in EPIC utilization.
  • Assists with registry audits or registry adjudications when requested by national registries.
  • Tracks patients and collects follow-up data as appropriate.
