Clinical Pharmacist--Centralized Refill Provider

We are a non-profit, FQHC-LAL providing medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. Southwest Care Center is currently seeking a Clinical Pharmacist for the primary functions of serving as our centralized refill provider for our clinics in both Albuquerque and Santa Fe, NM. This position is remote hybrid with some on-site work required in both locations.
Practice Highlights:
- Hours of Operation: Monday - Friday, typically 8a-5p
- System-wide Athena EMR
- Onsite care team includes Physicians, Nurse Practitioners, Clinical Pharmacists, Behavior Health Clinicians, Case Managers, RN Clinic Managers, RNs, Medical Assistants, and Diabetes Educators

Candidate Highlights:
· Two years relevant pharmacy experience
· Doctor of Pharmacy degree or equivalent
· Valid Registered Pharmacist license from the New Mexico Board of Pharmacy
· Valid Pharmacist Clinician license from the New Mexico Board of Pharmacy
Essential Duties:
1. Responsible for the review and processing of prescription refills requests via phone, fax, or electronic transmission for clinic providers across multiple clinic locations.
2. Responsible for transmitting prescription refill request approvals or denials to pharmacies via electronical health record software and including appropriate documentation.
3. Assists clinic providers in creating and managing the therapeutic drug regimens of patients with chronic disease states within the professional area of expertise.
4. Reviews medications with the patient, including dosing schedule, potential side effects, drug interactions, resistance patterns, therapeutic duplication, safety, efficacy, and adherence to the most up-to-date treatment guidelines.
5. Performs electronic medical record maintenance (medication reconciliation, review and update allergies, reconcile past medical history and problem list as appropriate, update routine health maintenance section as appropriate).
6. Performs physical assessment of the patient, orders laboratory and diagnostic tests as clinically necessary (including screening tools and vital signs).
7. Performs assessments to guide treatment, such as cardiovascular risk score, renal function calculations and dosing considerations, osteoporosis risk score, urine fractions excretion of phosphorus, pregnancy safety and dosing considerations.
8. Assesses and address any patient social determinants of health and other barriers to care, referring patients to case management services as appropriate.
9. Discusses adherence to enhance therapeutic success and avoid adverse drug reactions or adverse therapeutic outcomes.
10. Provides educational materials and counseling concerning medications, monitoring, and adherence strategies.
11. Ensures patients have follow-up with provider and schedules acute care or follow-up visits with providers as necessary; makes referrals as necessary (e.g., behavioral health, clinical pharmacy, diabetes education, etc.).
12. Orders labs if necessary for assessment or treatment monitoring.
13. Uses motivational interviewing strategies to assess patient readiness for medication therapy and encourage behavioral change.
14. Communicates with clinic providers and multidisciplinary clinic team as needed to ensure excellent patient care and safety.
15. Evaluates the appropriateness of prescription alternatives on health insurance plan formularies and adjusts prescriptions as necessary.
16. Communicates with prescription drug prior authorization team to facilitate prior authorization processing and submission of required elements of medical record.
17. Documents assessment, plan, and recommendations in the patient’selectronic medical record, and bill insurance or grant fundingstream as appropriate.
18. Follows up with teams in our dispensing pharmacies as needed to coordinate medication refills, promote seamless access to medications, and avoid interruptions or delaysin medication therapy.
19. Participates in quality assurance and compliance initiatives as they relate to clinical pharmacy or primary care services.
20. Supervision of pharmacy technicians and pharmacy interns as appropriate per licensure.
21. Participates in precepting college of pharmacy students as well as serving as a SCC PGY1 community-based pharmacy residency preceptor.
22. Consultant pharmacist duties, if assigned, such as policy and procedure development, licensure, and regulatory oversight of SCC clinic drug rooms to ensure compliance with the Board of Pharmacy rules and regulations.
23. Understanding, adherence to, and compliance with state and federal laws,DEA, Board of Pharmacy, SCC Policies and Procedures, and Pharmacist Clinician Protocol.
24. On-call duties on a rotating schedule with other pharmacists, if assigned.
25. Travel for training, patient care, and staff coverage as directed.
26. General knowledge of an ability to perform staff pharmacist and pharmacy technician duties in the case of an absence or crisis.
27. Other duties as assigned.
