This position is responsible for providing comprehensive, individualized case-management services to participants of North Side Housing and Supportive Services (NSHSS) Day Support Services Center, which is a drop-in program that serves individuals experiencing homelessness. Responsibilities include entering individuals into the Coordinated Entry System, conducting intakes, and connecting clients to community resources. This position requires working out of our Ravenswood office location and our Emergency Shelter located in the Rogers Park neighborhood.

Overview of Essential Functions:

  • Provides drop-in case management services to individuals experiencing homelessness.
  • Works to engage participants in the program with the intent of stabilizing clients and securing housing.
  • Complete NSHSS assessment with clients using established clinical tools and NSHSS protocols and determine appropriateness and eligibility for program and additional services
  • Matches clients to appropriate ancillary services (i.e., medical care, mental health services, employment services, food) which provide appropriate care in a geographically and culturally accessible way.
  • Determines the ongoing relationship between clients' clinical issues, service needs, and stable housing options and develops appropriate service plans based on that assessment.
  • Performs crisis prevention and intervention as needed. Reports critical client care issues or service provider issues to direct supervisor.
  • Maintains clients’ files. Performs written and electronic recordkeeping as required by internal and external funders.
  • Travels to conduct case management in the field, or to conduct Outreach and Engagement within the community

Skills/ Competencies:

  • Familiarity of case management databases such as HMIS.
  • Creates and maintains a positive and proactive work environment
  • Ability to manage multiple responsibilities, projects, and deadlines concurrently.
  • Ability to work in an interdisciplinary team approach model, taking individual responsibility and autonomy while also working well in roles leading and serving as part of a team.
  • Strong written, organizational, communication and presentation skills.
  • Ability to both work in an office environment, out in the field, or remote setting.
  • Flexibility that enables one to meet the ongoing needs of participants and the agency.
  • Ability to use windows-based computer applications including Microsoft Word, Excel, Power Point, SharePoint and Adobe Reader.

  • Interpret and apply clinical criteria and screening results regarding client and form an appropriate service plan
  • Work effectively with clients one-on-one or in groups
  • Develop appropriate clinical relationships with clients of different cultures, races, religions, sexual orientations and gender identities
  • Communicate effectively verbally and in writing
  • Represent NSHSS in a positive and professional manner
  • Identify client needs and refer to appropriate ancillary service providers
  • Use confidentiality rules and regulations and apply to daily work situations
  • Make appropriate clinical decisions based on NSHSS criteria

Knowledge:

  • Of the Continuum of Care and Chicago Plan to End Homelessness.
  • Of various clinical criteria related to issues confronting our clients (i.e. substance use, mental health treatment, health insurance, etc)
  • Of local service provider network
  • Of NSHSS criteria
  • Of applicable state and federal confidentiality regulations
  • Of program rules and standard operating procedures
  • Of basic computer functions and the management information system
  • Of motivational interviewing principles
  • Of harm reduction principles
  • Of the Housing First Model

Qualifications:

  • Commitment to the mission of North Side Housing and Supportive Services.
  • Bachelor’s degree in a related field OR three to five years of equivalent experience in a human services/behavior-related field preferred.
  • Endorses a Trauma-Informed, Strength-Based practice model that integrates Harm Reduction and Motivational Interviewing.
  • Experience working with the homeless population preferred as well as with individuals with mental illness and/or substance use
  • Spanish language proficiency a plus.
  • Intermediate computer skills.

Other Responsibilities:

  • Sort and organize donations
  • Assist agency clients with accessing the food pantry, clothing closet and household goods shop
  • Participate in scheduled staff meetings
  • Attend training and apply skills learned to NSHSS service delivery system
  • Participate in quality assurance activities (i.e. file reviews) as requested
  • Keep informed of all policies, procedures and regulations that apply to this position
  • Other duties as assigned

Pay: $38,000.00 - $42,000.00 per year
