Position Overview
This position provides services to youth based on the High Fidelity Wraparound model of care. The Youth Support Partner (YSP) will meet with youth in their homes, agency, and community settings, and will work collaboratively with the Care Coordinator, treatment team, and community members. Must be able to work flexible and on-call hours.
Summary of the Essential Duties and Responsibilities
- Services necessary for the continuity of client care (consultation, supervision, referrals, phone messages, correspondence).
- Weekly meetings with the Wraparound Facilitator
- Participation in promoting High Fidelity Wraparound in the community.
- Attendance, as assigned by the supervisor, of High Fidelity Wraparound conference calls and related activities.
- Public speaking.
- Location of clients by phone contact & home outreaches.
- Assistance in assessment of families' needs and connecting them to relevant community resources.
- Participation in Wraparound meetings.
- Assist families in identifying and utilizing natural supports
- Prepare families for meetings and/or attend meeting with them (i.e. school, CPS)
- Meeting with family members outside of the Wraparound meetings to support, encourage, and plan.
- Collateral meeting with counselors, counselor assistants, PCPs, other medical providers, school staff, and Crisis Response Services as needed.
- Maintain availability to meet with families both in and outside of the normal business day (i.e. evenings).
- Serve as a mediator, facilitator, and cultural broker between youth and agencies.
- Utilize own lived experience and connection to communities and the peer movement to bring resources and informal supports the CFT.
- Ensure each youth is heard and their individual needs are being addressed and met.
- Demonstrate leadership experience and diplomacy in resolving conflicts and integrating divergent perspectives.
- Build relationships with community members and organizations to connect the youth with resources.
- Share resources and information in a developmentally appropriate way to ensure that the youth understand the WISe process and have access to information regarding their care.
- Communicate with and educate agency staff on the importance of youth voice and choice and the power and benefits of peer involvement, particularly in transition age youth.
- Serve as a peer advocate to help empower youth in gaining the knowledge and skills necessary to be able to guide and eventually drive their own treatment.
- Effective use of technology and software including but not limited to: computers, phones, email, EMR, etc.
- Ability to work with difficult cases and vulnerable populations.
- Be helpful, respectful, approachable and team-oriented.
- Effective time management including but not limited to: planning ahead where possible and be punctual in person and work performed.
- Build strong working relationships and a positive work environment.
- Be receptive to feedback, willing to learn, and embrace continuous improvement.
- Complete all documentation, reporting, and other paperwork per agency standards.
- Attend all appropriate staff, organizational, and external meetings (as assigned).
- Work efficiently both independently and as part of a team.
- Other duties as assigned.
Minimum Qualifications:
- HS Diploma or equivalent is required
- Where applicable, the YSP must be WISe trained and complete Peer Certification within 6 months from date of hire
- YSPs must demonstrate the ability to communicate effectively and relate with families in a positive, respectful manner
- Registration as an Agency Affiliated Counselor in Washington State is required upon hire.
- Valid driver's license.
- Able to pass a multi-state background check.
- Must have lived experience as a youth of intensive behavioral health services; this individual will have sufficient time in their own recovery and be able to utilize their personal and professional experience in order to provide peer support and teach coping skills.
Preferred Qualifications:
- College coursework in the counseling or social science fields is strongly preferred.
Salary: $18.38/hr
