Be a part of The Community Health Team and contribute to the objectives of the Blueprint for Health’s Women’s Health Initiative of decreasing unintended pregnancies and improving family well-being. The Women’s Health Initiative (WHI) Care Coordinator supports the workflow of screening patients for needs related to social determinants of health, mental health, and substance use. The Care Coordinator then provides the care coordination, brief intervention and referrals to resources needed to support patients in achieving their health goals. The ideal candidate is someone who has found their calling in the social work field, enjoys working with those who can become pregnant and their families, and is passionate about collaboration to provide care that takes care of the whole person.

Functions of the position include:

A. Coordinates patient care with members of the integrated health care team.

1. Receives referrals for patients with positive screening results for social, mental health, or substance use treatment needs. Complete additional screenings as needed.

2. Assesses the patient’s barriers and readiness for change using motivational interviewing techniques and build a therapeutic relationship and establish goals.

3. Make and follow up on referrals to community resources to support the patient’s goals.

B. Serves as the Close-to-Home contact for pregnant people recovering from opioid addition.

1. Acts as the liaison between the prescriber of buprenorphine and The Women’s Center.

2. Ensures appropriate follow up appointments are in place and communicates the care plan with the patient and care team.

C. Provides brief intervention mental health support.

1. For patients with immediate mental health needs, provide resources and coping tools.

2. Provides brief (30 min) solution- focused behavioral interventions with active development of Behavioral Change planning.

3. Provides follow up appointments as needed and consistent with a Brief Intervention model.

4. Set goals with the patient for obtaining longer-term mental health care and make a referral to a mental health provider.

5. Conduct CALM as needed for patients with thoughts of self-harm.

6. Provides on-site availability for crisis triage, suicide screening and intervention. Provides crisis intervention and utilizes the collaborative assessment for managing suicide (CAMS).

7. Provides quality clinical consultation to providers and staff as needed in areas of individual, family, child and adolescent regarding mental health and substance misuse related issues that are directly connected to the patient’s medical condition.

Educational requirements: A clinical mental health master’s degree. Mental health license preferred, willing to become licensed required. Three years of specialized experience has provided the applicant with specific knowledge, skills, and abilities to successfully perform the duties of the position. Preferably experience in a primary care office, or equivalent in meeting chronic care and mental health needs of patients.

Job Type: Part-time

Pay: From $30,000.00 per year
