Job Description-

Reformation Home Health Care is currently hiring for a On Call/Weekend Warrior Direct Care Coordinator (Home Health Aide) in the FLINT area.

Position Details:

* Every weekend

• DIRECT CARE COORDINATOR IN THE FLINT AREA
• Continuation of care with this same participants (patients) every shift
• Pay: STARTING at $15.00 an hour. Pay raises available pending work ethic after 30 days.

Position Summary:

Responsibilities include but not limited to

• Homemaking services: Support participants by assisting with cleaning bathroom, vacuuming, washing dishes, dusting and laundry services, etc.
• Personal Care: Support participants by assisting with bathing, toileting/brief changes, dressing, grooming
• Activities of daily living: preparing and serving meals and snacks, running errands/shopping with participants
• General health care and monitoring: overseeing medication and prescriptions usage, appointment reminders and administering medicine. Following a care plan and noticing any changes in the individual’s health, recording and reporting any differences
• Mobility assistance: help with transferring in and out of a wheelchair, car or shower
• Personal supervision/Respite: providing constant companionship and general supervision; providing other caregivers a break
• Transportation: driving to and from activities, running errands, and help getting in and out of wheelchair-accessible vehicle
• Emotional support: being a stable companion and supporter in all matters personal, health-related and emotional
• Care for the elderly: orienting or grounding someone with Alzheimer’s disease or dementia, relaying information from a doctor to family members

Position Requirements:

• Must be of 18 years of age
• Preferred one year verifiable Direct Care experience
• High school diploma/GED (CNA Preferred)
• A valid Driver's License or ID
• Reliable transportation (Must provide a copy of Vehicle registration)
• Weekends and holiday rotation
• Must be able to pass a background check
• Strong patient service skills
• Caring and compassionate personality
• Ability to work with limited supervision
• Ability to follow written and verbal instructions
• Strong verbal and written communication skills
