Hospice Nurse Case Manager - RN

** $7,500 Sign-On Bonus**

The Hospice RN Case Manager establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on care, on-going clinical management and education regarding hospice care.

As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery. Additional responsibilities of our Hospice RN include:
Making initial nursing evaluation visit.
Continually evaluating the patient’s needs
Initiating, coordinating and revising the treatment the treatment plan
Rendering nursing care and performing treatments, as ordered by the physician
Participating in the Quality Assurance/Quality Improvement Process
Preparing accurate, orderly and timely reports and documentation
Adhering to all practice standards as they apply to patient care

Requirements:
As a Hospice RN, you must be a caring, patient and flexible teacher with excellent communication and time management abilities. You must possess independent problem solving abilities, excellent clinical assessment skills and be computer literate.

You must also be in good physical shape and have the ability to lift/transfer patients to and from wheelchairs, if necessary.Additional requirements of our

Hospice RN include:
Associate’s of Science degree in Nursing
Valid Ohio RN license
Minimum 2 years of nursing experience including hospice or home health experience preferred
Valid driver’s license and one year clean driving record
Excellent writing, communication, and charting skills

Job Types: Full-time, Part-time, PRN

Pay: $68,000.00 - $74,000.00 per year
