Are you interested in helping patients in their homes? Are you a registered nurse with home health experience? Well, you are a perfect fit for our company!

We are looking to hire an RN who is available to do home visits for our patients.

Duties include but are not limited to:

-Make an initial evaluation within 48 Hrs of the patient's D/C from hospital or Physician's Order to assess the needs of the patient prior to the assignment of the Physical Therapist and Occupational Therapist or Speech Therapist. All Assessments and evaluations must be charted.

-Establish a Plan of Care to meet those needs; reassess the patient's needs and develop a new plan of care when needed.

Must be a licensed Registered Nurse

Home Health experience preferred

Valid CPR

Valid Driver's License

Must be able to pass a Family Safety Care registry, Sex Offender and OIG background screening
