• Assess admitted patients for clinical psychosocial financial and functional needs to create a safe discharge plan
• Develop discharge plan with the interdisciplinary care team identifying and initiating needed post acute care referrals coordination and collaboration of care need delivery
• Assess and utilize payer knowledge to secure coverage for needed service; initiate referrals for additional service coverage as needed;
• Collaborate with Utilization Management for coverage of acute care needs and post acute care needs
• Employ Motivational Interviewing or Shared Decision Making skills to identify patient/family goals establish viable d/c plans
• Communicate effectively with unit team and CM team regarding care plan changes financial challenges service deficits
• Evaluate readmission reasons for each patient and incorporate alternate options in d/c planning w/ a goal to decrease unplanned readmissions
• Participate actively in patient experience initiatives to improve preparation for discharge and knowledge of after care management

COMPENSATION

• $2,350 per week ($1,162 is untaxed per GSA per diem)
• $9,400 per month
• $65 per hour

• Current RN license in New York
• Current BCLS certification Required
