JOB SUMMARY:

The Qualified Mental Health Professional II- Adult (QMHP II-A) provides training and supports to individuals who have Serious Mental Illness (SMI) to achieve and maintain community stability and independence in the most appropriate and least restrictive environment, to include but not limited to, Mental Health Skill Building, Psychosocial Rehabilitation, and Ambulatory Crisis Services. Settings where services are provided include, but are not limited to, home, school, nursing home, or crisis center. QMHP II-As are responsible for individuals' related documentation and reports and/or the implementation of treatment goals and objectives, as assigned.

ESSENTIAL FUNCTIONS:

  • Carry an assigned caseload of Serious Mental Illness (SMI) individuals who require intensive training and supports to function independently in the community and/or maintain records and data within timeframes to meet regulatory standards, as assigned. Examples of caseload work include providing training in social skills, medication management, personal hygiene, financial management, household, cooking skills, and recreation.
  • Provide ongoing assessment of the needs and participate in the development of an individualized services plan that promote personal rehabilitative life goals and ongoing management of negative symptoms of SMI, utilizes local community resources, involves individual’s significant others (with properly authorized releases), and ensures coordination of services with other providers of services ( i.e., outpatient counselor, psychiatrist, crisis stabilization, etc.). Assist individual directly to locate, develop, or obtain needed services, resources, and appropriate public benefits. Provide transportation to and from scheduled appointments, social and recreational activities, shopping trips, and other Agency programs.
  • Provide symptom and behavior management, and individual and group counseling, to provide treatment goals that are implemented to avert hospitalization or re-hospitalization.
  • Know and monitor individual’s health status, any medical conditions, and his or her medications and potential side effects, and assist the individual in accessing primary care and other medical services, as needed, in order to avoid re-hospitalization. Provide crisis interventions, as necessary.
  • Effectively work with other Agency staff to include coordination with case managers, participate in team meetings, and provide training and support to Qualified Paraprofessionals in Mental Health-Adult (QPPMH_As).
  • Provide temporary intensive services to avert emergency psychiatric hospitalization or institutional admission or to prevent out of the home placement. Provide training to individuals, family members and other caregivers and service providers in a person-centered approach to assist the individual avoid hospitalization and remain in the community.
  • Assess, evaluate, and monitor the safety of the individual and others. Initiate and facilitate de-escalation techniques and strategies to assist the individual in maintaining self-control.
  • Maintain required certifications and attend mandatory training

OTHER DUTIES:

  • Must be willing to work hours, which coincide with the person’s needs, to include evening, night, and weekend hours.
  • Responsible for monitoring car maintenance needs and observing safety protocols when transporting individuals.
  • Perform such other duties as assigned by supervisor.

QUALIFICATIONS:

  • Ability to interact with people in a supportive/person-centered and therapeutic manner.
  • Knowledge of the nature of mental health, developmental disability, and substance abuse disorders.
  • Ability to work as part of a team.
  • Good oral and written communication skills.
  • Financial and money management skills. Knowledge of resources and criteria for eligibility.
  • Must be able to work independently and with supervision available at broadly spaced intervals.
  • Knowledge and skill in completing comprehensive assessments, data collection, developing and implementing service plans for SMI individuals and medications used to treat SMI. Must be able to maintain records and data within timeframes to meet regulatory standards.
  • Requires a high level of self-motivation and accountability to individuals, program, and the Agency.
Minimum Requirements

EXPERIENCE/EDUCATION REQUIRED:

A Bachelor’s Degree in human services field.

Must meet Board of Health Professions registration requirements for Qualified Mental Health Professional (QMHP-A) or Qualified Mental Health Professional (QMHP-A) Trainee. See http://www.dhp.virginia.gov/counseling/counseling_QMHP.htm for further information.

Valid Driver’s license and safe driving record required.
