Cherokee County Public Health seeks a Care Coordinator Liaison to focus on building and strengthening our network of partners in order to increase the connection to medical and dental homes for underserved communities and increase access to local counsel for uninsured individuals. This work will include building relationships with a network of organizations to explore future partnerships, including referral partnerships and partnerships focused on outreach and information distribution.
Schedule:
Benefited, Full time, 40 hours weekly, flexible scheduling ( some hours may be outside of normal business hours)
You will:
- Develop and update medical informing plans and care coordination.
- Build relationships with previously and newly identified organizations within the service area, in order to increase equitable access to medical and dental homes.
- Work with Cherokee County Public Health’s team to create a network of medical and dental providers, with a goal of establishing a relationship-based referral network across the service area.
- Maintain the medical and dental provider referral database for the service area.
- Coordinate collection of information (through surveys, Google forms, emails, focus groups, individual meetings, etc.) from referral network and other community and partner organizations to ensure two-way communication between partners.
- Assist with clinics and other client services as requested.
- May perform basic office tasks such as filing, calling patients, scheduling appointments and other office duties within the Public Health office.
- Work with the public, exhibiting courteous behavior, non-judgmental attitude, being culturally aware and maintaining client confidentiality.
- Other duties as assigned.
To thrive in this position, you should be comfortable in talking on the phone in promotion of medical and dental services to individuals across the lifespan but especially to children. You should have the ability to travel within the assigned region. You should have the ability to communicate effectively and have a proactive attitude toward finding and recruiting new partners for medical informing and creating and maintaining a referral database. You must also have a willingness to learn about our efforts in making a difference. Ability to easily build rapport with communities by utilizing active listening and interpersonal skills. Team-oriented individual with the ability to problem-solve independently. Strong organizational skills.
To be considered for this position, you are required to have a High School Diploma or GED.
Travel may be required. The individual hired must have a valid driver’s license and have the ability to travel from one location to another within the service area on their own.
