General Description:

The Population Health Analyst is responsible for assuring the validity and reliability of electronic tools for population health management (PHM). Electronic tools include but are not limited to daily huddle reports, HRSA UDS clinical quality metric dashboards, patient outreach registries, and ad-hoc reports. This role’s duties are foundational to quality management at Primary Health Network.

Location and Hours

The Primary Health Network Sharon, PA. (NOT a remote position)

Monday - Friday 8:30AM to 5:00PM


Duties:

  • maintain awareness of all relevant aspects of data context
  • construct report criteria that are logically aligned with approved clinical guidelines
  • keep all PHM metadata records up to date
  • keep the reporting element table clean and aligned with metadata requirements
  • update report criteria to align with contextual or other changes as needed
  • monitor the production quality of associated EMR data and provide timely alerts of findings to team
  • troubleshoot reporting software functionality issues as they arise and provide timely alerts of findings to team
  • complete other validity assurance tasks as needed
  • Work with all necessary internal and external information stakeholders to clearly define the intended report function
  • Create realistic informational and turnaround expectations
  • Construct a report output that aligns report design with intended function and clearly communicates findings
  • Provide report output interpretation guidance to minimize misinterpretation
  • Ensure that reports are available to all necessary stakeholders
  • Complete other reliability assurance tasks as needed
Education Requirements
  • Bachelor’s degree in nursing, healthcare information technology, public health, or a field within the life or social sciences

Job Requirements/Qualifications


  • One or more years of full-time experience producing and analyzing complex reports from large datasets using Excel formulas and tables or a SQL-based application
  • high level of attention to detail, results-driven
  • ability to work on multiple complex projects at a time, onsite as needed
  • committed to the purpose of community health centers (FQHC) and other safety-net health care provider organizations that provide care regardless of ability to pay
