SCHEDULE: THREE 12-HOUR SHIFTS PER WEEK FROM 6:00PM-6:30AM, EVERY THIRD WEEKEND

JOB RESPONSIBILITIES:

Assists with admission process by taking initial vital signs, measurements and weights, orienting patient/family to unit, hospital facilities and services, and documenting on appropriate forms per unit standards.

Assist nursing to deliver patient care according to unit standards, policies and procedures, as well as patient’s plan of care. Contributes relevant and appropriate information to RN. Maintains confidentiality of patient related issues. Supports the Patient Bill of Rights and Hospital Mission and Values.

Participates in meeting and reinforces patient’s educational needs, documents accordingly.

Demonstrates knowledge of sterile technique.

Assist nursing to monitor and accurately record patient intake and output. Reports changes in patient’s status (e.g., changes in vital signs, level of consciousness, weight, mobility status, comfort level, feeding pattern or any unusual occurrences) to RN in a timely manner.

Under direction of RN, assist patient with routine activities of daily living: feeding, bathing, ambulating and skin care according to policy and documents appropriately.

Demonstrates effective interpersonal skills appropriate to the age, type and development level of patients.

Answers call lights timely and intervenes appropriately. Responds to patient and family requests promptly.

Transports patients to various departments and upon discharge.

Stocks and orders supplies for unit and/or patient.

Removes equipment and supplies not in use; cleans equipment and returns to appropriate storage location.

Keeps patient area clean and free of trash and clutter. Keeps linen appropriately stored; empties linen hampers and trash as needed.

Provides and documents patient treatments.

Provides emotional support to patients and families; documents appropriately.

Attends and participates in unit related in-services, conferences and meetings.

Collaborates by working with others to achieve unit and organizational goals.

Utilizes proper chain of command for resolution of difficult issues.

Participates in unit staff meetings and CQI plan to improve organizational performance and achieve unit and organizational goals.

EDUCATION, EXPERIENCE, AND TRAINING: High school diploma or equivalent. Certification as a Nursing Assistant or equivalent on the job training specific to the area for a minimum of three months. Maintains current CPR certification.
