Job Summary and Qualifications
The Patient Care Coordinator is responsible for the programs and resources that support the coordination of Emergency Department patients through complex healthcare environments for the optimization of care, outcomes, and patient experience. Responsible for facilitating patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The Patient Care Coordinator guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes while consistently providing exceptional customer service. The Patient Care Coordinator drives and fosters a culture of patient and family-centered care and service excellence while improving the patient and family experience and patient relations.
- Reviews patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention
- Communicates with physicians (in the hospital and in the community) about patients' potential for referral / follow-up treatment
- Schedule appropriate referrals as noted by the physician
- Coordinates, investigates and resolves consumer grievances concerning the quality of care complaints by providing a formal grievance mechanism for the hospital
- Acts as a liaison between patients, their families, community, physicians, and hospital staff
- Understands problem resolution processes
- Directs the effort to collect, analyze, and evaluate consumer satisfaction data
- Utilizes feedback to facilitate systems improvements to better meet the consumer's needs
- Assists with discharge call backs to patient
- Supports the accountability scorecard process, e.g. process metrics, patient verbatims, close the loop
- Distributes comments to appropriate parties
- Provides tactic follow-up tasks
- Participates in Nurse Leader Rounding
- Promotes confidentiality, respect and dignity for all persons as standards for department and hospital
- Train managers/staff on website use and answer questions regarding use of the Press Ganey website and survey process
- Completes other duties as assigned
What qualifications you will need:
- Associate’s Degree and/or completion of medical training program (LVN, EMT, Paramedic, Therapist, Counselor, etc.)
- One (1) year customer service, customer relations, patient experience or advocacy experience
- Effective communication, relationship building and follow through skills
Preferred skills:
- Bachelor’s Degree
- Nursing or clinical background
- Experience in the Medical Office Setting
- Knowledge of the health care delivery system
