Brookside Healthcare and Rehabilitation Center, a 120 bed beautiful facility, recognizes and respects, that Brookside is home to our residents. Therefore, we take a personalized approach in caring for our residents, providing comprehensive medical, emotional, and social support. We are searching for experienced, caring, & dependable applicants to join our TEAM on a part-time basis.
- Assures that notes are informative and descriptive of services provided and of residents’ responses to services
- Prepares and completes social service documentation in timely manner
- Develop a social history on each resident
- Complete sections of the MDS
- Assists with addressing resident concerns and searching for a positive resolution
- At times, participates in the Utilization Review and other committees as directed and needed
- Develops discharge plans at the point of admission to provide direction for the resident's stay
- BSW or MSW
- Knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
- Previous long-term care experience is desirable. Past supervisory experience preferred.
