We are looking for a Case Manager – Mental Health Professional (MHP) who exemplifies that mission, and who wants to work for an organization that makes a difference. Are you the right fit?

What will you be doing?

• Developing and implementing individual programs to maximize the clients’ independent community living skills
• Completing daily documentation of mental health services provided to clients
• Working with a team of behavioral health service providers within the agency to ensure a positive client-centered approach

What will you bring to the table?

• Bachelor’s degree in human services or a related field (required)
• Experience working in social services (required)
• A valid Illinois driver’s license and automobile insurance
• Navigation of stairs
• Ability to lift 50 lbs. or more
• Ability to provide services in a client’s home
• The use of your personal vehicle or agency vehicles to transport clients
• Maintaining required certifications and other relevant trainings
