We are seeking a highly organized and detail-oriented MDS Coordinator to join our team. The successful candidate will be responsible for managing the Minimum Data Set (MDS) process in our long-term care and rehabilitation center. This includes ensuring accuracy of all MDS assessments, providing guidance to nursing staff, and ensuring compliance with all applicable regulations. The ideal candidate has experience in a nursing home or other long-term care setting and is familiar with the MDS process.

Responsibilities:

• Monitor the quality of care provided to residents in the facility.
• Ensure that all nursing staff are properly trained and certified.
• Develop and implement policies and procedures for the facility.
• Maintain accurate records of resident care plans and progress notes.
• Coordinate with other departments to ensure that all services are provided in a timely manner.
• Provide guidance to staff on clinical issues and resident care plans.
• Ensure that all regulatory requirements are met.

Job Types: Full-time, Part-time, Temporary

Pay: $32.00 - $33.00 per hour
