JOB RESPONSIBILITES (include, but not limited to the following):

• Provide skilled nursing services in accordance with the orders of a physician and under a plan of care in a patient’s home.
• Maintains communication with attending physician, medical director and other personnel of changes in the patient’s needs and outcomes of intervention.
• Implementing a plan of care which emphasizes the teaching of based practices. This person will be working closely with the clinical supervisor to coordinate patient care.

QUALIFICATIONS:

Graduate of a school of professional nursing and is currently licensed, certified, or registered in the state(s) in which he/she practices. Minimum of one (1) year of experience in general nursing practice, and home health care. Should have good communication skills. CPR certification and valid driver’s license. Ability to work well with others, function well under pressure, and function independently.
