HEALTH SERVICES DIRECTOR/LPN– Grow your career with Merrill Gardens!

In this position the HSD will oversee our Assisted Living and Memory Care. The position will include doing care plans, working with the Contract RN, (as needed) changes of condition, 90 day re-evaluations, wound/skin checks, psychotropic reviews, behavior plans, (if needed) communicating with families about care plans and changes that may be needed with the resident, etc. You will be working closely with the Executive Director to make sure we are meeting the needs of the residents and giving them the best care!

This does require being on call if PRN's are needed in MC. The Resident Care Director and Garden House Supervisor handle all staffing, scheduling, care conferences, etc.

We have Senior Leads, MC Supervisors and a Resident Care Director who provide so much support and experience to the team.

We are looking for someone who is experienced in Assisted Living, a team player, loves working with Seniors, organized, detail orientated and most of all- has a heart to love and care for seniors.
