Program Description:

Catholic Charities OMI Senior Center helps seniors maintain their independence by providing activities and classes in a safe environment during the day. Seniors receive hot meals, guidance, and support as well as referrals to additional services available throughout San Francisco.



Job Description:

Under the supervision of the Program Director, the Information and Assistance Specialist assists in the navigation of the home and community-based long-term care system; provides assessment, information, referral and assistance to older adults, disabled adults, caregivers, individuals and agencies; performs follow-up to ensure delivery of services; collects, updates and shares information on community resources, and completes outreach activities as assigned.



Job Duties & Responsibilities:

  • Conducts intake and needs assessments of all clients in order to provide them with information and/or referrals to agencies, resources, or services that match their needs.
  • Assists clients in accessing needed services and advocates on their behalf when the requests do not require short-term casework level of assistance. (e.g., contacting agencies on behalf of clients, filling out forms, writing letters, or making phone calls to set up or confirm appointments).
  • Provides clients with information and/or translation services and assist with forms as needed.
  • Provides referral follow-up confirming that clients obtained actual services.
  • Establishes contacts and maintains good working relationships with key personnel in other agencies, including community-based service providers in the district and citywide providers.
  • Provides information and promotes utilization of services by conducting outreach to businesses, groups and individuals; schedules and makes presentations to various target groups two or more times a month.
  • Attend resource fairs, and ADRC Partnership meetings assigned by the Program Manager.
  • Attends trainings to enhance skills in working with the seniors and adults with disabilities.
  • Works closely with ADRC volunteers and student interns to extend their outreach and support to clients.
  • Participates in professional development trainings as assigned by the Program Manager.
  • Completes proper documentation, data entry of reports, and submit monthly service logs and reports as needed.
  • Provides telephone and office support, including outstations at other agencies as needed.
  • Makes recommendations to enhance and improve services.

Qualifications:

Education & Experience:

  • Bachelor's degree in Social Work, Gerontology, or related field highly desirable. or
  • AA Degree and three years related work experience in social services in a community-based social service setting.
  • Experience in providing health, social, and/or aging services in the community, especially with diverse communities and knowledge of community resources.
  • Leadership abilities, teaching experience, ability to take-charge and/or make decisions as needed.


Knowledge, Skills & Abilities:

  • Ability to relate to seniors, people with disability and persons in crises.
  • Knowledge of structure and content of services to seniors.
  • Ability to work independently in a culturally diverse setting/community.
  • Promote a positive professional and organizational image in the community.
  • Solid written and verbal communication skills.
  • Good problem-solving skills and ability to follow through.
  • Strong interpersonal, communication, and telephone skills
  • Computer literacy.
  • Second language skills in Tagalog, Cantonese, Mandarin and Spanish required.:
  • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross-cultural situations.

Council on Accreditation (COA) Roles Is responsible for accurate and timely data entry that aids Performance and Quality Improvement (PQI) reporting and case record reviews.
