An Overview
The Human Services Specialist II, IDD Intake/Eligibility Waitlist Coordinator position requires frequent use of initiative and independent judgment; professional courtesy; in person, intermittent short-term contact with individuals, families and service providers and the ability to work with minimal supervision. Job responsibilities may require traveling within the service area and flexible hours and schedule.
Essential duties include completion of comprehensive assessments of individual needs and desires, service planning, referral and linkage, monitoring and advocating on behalf of individuals, documentation of services provided and interaction with the individual/family served.
An employee in this class receives oversight and case review from a supervisor. Client interactions are sensitive and require that employees maintain confidentiality of client records and treatment. The work performed in this classification does not require clinical licensure.
The Opportunity
- Receives requests for intakes and schedules intake appointments;
- Informs individuals of required documentation needed for the intake and subsequent appointments and assists them with obtaining documents when needed to expedite the process;
- Evaluate and assess client and/or family's service-related strengths, needs and preferences;
- In participation with the client, helps identify resource needs and appropriate waiver program for clients;
- Conducts eligibility assessments for Medicaid Waiver programs;
- Performs administrative support work associated with the intake process;
- May conduct home/site visits;
- Maintains communication with clients, family, support worker, teachers, and providers;
- Prepares reports concerning status of clients awaiting intake;
- Prepares and maintains client files using an electronic health record system;
- Makes referral of clients to other agencies;
- Prepares intake forms and routine correspondence, such as follow up letters to clients;
- Performs related work as required.
Schedule: Monday through Friday, 8:00AM – 4:30PM, flexible hours to accommodate needs of the customer. Not to exceed 40 hours per pay cycle. Telework or compressed schedule possible upon completion of probationary period.
