Supportive Services for Veteran Families: LIFT Housing Case Manager
Position Description:
Over the last 68 years, SVdP has grown from a volunteer-led non-profit to the largest human services Since 1988, St. Vincent de Paul has developed multi-family housing for low-income families, seniors and people with disabilities. We’ve developed more than 1,600 units so far and operate almost 1,500 of those units. The Onsite Manager performs Apartment Maintenance and basic office tasks necessary to ensure consistent operation of the affordable housing property.
Vet LIFT, 40 hours/week, this is a Full-time, benefits eligible position. Salary: $41,600.00 - $47,840.00 per year

Qualifications: BA/BS in Social Work or related field. Experience working with homeless Veterans, familiarity with local VA, social service providers and community resources required. Familiarity with client driven case management required. Ability to work effectively with other program staff and coordinate with other service agencies. Excellent written and oral communications skills required. Strong problem solving and mediation abilities required. Veterans and bilingual/bi-cultural persons are urged to apply.
Responsibilities will include the following, with other duties as assigned.
Selection: Responsible for working with a team comprised of VA representatives and Vet LIFT staff for appropriate selection. Coordinate with SVdP Property Management Department to insure all rental processes are completed.
Intake: Responsible for intake of veterans entering the Vet LIFT program. This intake will include all demographic information, service history, education, employment, housing, service access history, and general background.
Assessment: Responsible for developing a comprehensive assessment with each veteran on case load. Assessment will include each veteran's assessment of their own skills and challenges, associated service provider’s assessments, and case manager’s assessment. Responsible for periodic reassessment of each veteran to insure continuing relevance of services.
Case Management: Responsible for a case load of individuals/families in Vet LIFT’s Housing Programs. Will be responsible for working with each Veteran to develop case plans, develop service access strategies appropriate for each plan, and developing team case management with other agencies associated with each Veteran. Conduct home visits, assist with financial planning, and coordinate as needed with housing department. Responsible for all record keeping associated with each case.
General: Record keeping, maintenance of eligibility documentation, case records, service access coordination, VA data and Service Point data entry and other case plan documentation.
