Hands of Comfort Hospice is a brand-new hospice servicing Maricopa County that is owned and operated by licensed healthcare providers and nurses. We are passionate about compassionate patient centered care and changing the standard for excellence in the Hospice care setting and are seeking like-minded individuals to fill all interdisciplinary team roles. We are currently looking for PRN Chaplains/bereavement coordinators to join our team. This position is PRN paid at a per visit rate of $35.00 to $50.00 (based on experience). The position does have the potential for full time as census grows.

The Chaplain serves an invaluable purpose to the patient and family during end-of-life care.

You will be responsible for the spiritual/support counselingservices provided to hospice patients and families. Chaplain will serve as liaison and resource for community spiritual leaders in dealing with hospice patient/family. The chaplain's responsibilities include performing religious rites, conducting worship services, and providing confidential counseling.

Responsibilities:

  • Assesses and identify the patient/family/caregiver spiritual status and needs.
  • Ensure that Hospice philosophy of care as defined by state and federal regulations is appropriately implemented.
  • Provide patient assessments at the location that the hospice services are delivered per regulations.
  • Provides pastoral and spiritual care services to patients/families/caregivers.
  • Collaborate with spiritual colleagues as appropriate.
  • Participate in interdisciplinary team meetings as needed.
  • Consult and collaborate with other members of the interdisciplinary team in the development and updating of the patient care plan.
  • Assist in finding clergy to perform funeral, memorial or graveside committal services as requested by families and assists with planning for services.
  • Assure that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality.
  • Adhere to all Agency, state and federal policies and procedures, laws, and regulations which are relevant to job responsibilities.
  • Maintain confidentiality of business and health information in accordance with HIPAA
  • Provide bereavement follow-up services as assigned

Qualifications:

  • Education: Bachelor’s degree from an accredited institution in theology, divinity, religion or equivalent required.
  • Experience: Professional pastoral counseling in healthcare, home care, hospice or palliative care setting required.
  • Minimum of two years experience providing spiritual support and counseling to clients and family members in a health care setting.
  • Demonstration of expertise in spiritual assessment and counseling, and in concepts of death and dying issues.
  • License/Certification: Licensed driver in Arizona, with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
