- Reports To: Clinical Manager
- Qualifications:

Master’s Degree from an accredited School of Social Work.

Current license to practice as a Licensed Social Worker in the State of New Jersey.

A minimum of two years experience under qualified supervision in a health care setting, one of which is Hospice and/or Home Care.

- Responsibilities:
- Performs initial psychosocial, environmental, safety, financial and emotional assessment with the patient/family/caregiver.
- Assists patient/family/caregivers in psychosocial, environmental, safety, financial, and emotional concerns and/or provides resources to assist family in those concerns as ordered by the physician.
- Develops plan of care with the case manager and physician.
