- Qualifications:
- Licensed as a RN issued by the state of New Jersey.
- BSN and two years acute and/or critical care nursing experience; OR equivalent combination of education and experience.
- Responsibilities:
- Provides direct care, support and education utilizing the nursing process to patients and families in their homes under the direction of a physician.
- Coordinated the case management among the multidisciplinary team.
