The In Home RN will ensure quality and safe delivery of home care services in 1:1 setting. Able to consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plan. To assist in the provision and coordination of all home care services in accordance with Agency philosophy and established standards of home health and nursing practice.
Responsibilities:
- Consults with the case Clinical Coordinator problems concerning the patient’s (clinical and non-clinical issues) and collaborates in problem-solving.
- Observing signs and symptoms outside of parameters, untoward reaction to treatments and/or medications, and significant changes in the patient’s physical or emotional condition.
- Monitors and oversees the delivery of daily care needs.
- Vent experience required.
Requirements:
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in the State of Michigan.
- Must possess and maintain current CPR Certification.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines.
- Previous experience in an assisted living, home health, hospital, long-term care environment, or other related healthcare field.
