Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for a Full Time FNS/RN Educator in Northern and Centeral New Jersey.

Key Functions Include:

• Institute contact with physicians as necessary.
• Confer with patient’s physician in ascertaining the need for home care and in developing, implementing and revising the Plan of Care.
• Perform initial patient assessment on those patients admitted to the agency for care.
• Develop an individualized goal outcome Plan of Care.
• Provide and supervise care according to the Plan of Care.
• Ensure that physician’s orders are obtained initially, reviewed as required and communicated appropriately.
• Conduct training programs for PCA’s and HHA’s.
• Assist with home maker and HHA supervision.
• Instruct in-service training as required by regulations and branch operations.
• Participate in orientation to policies and procedures for staff members.
• Perform initial and annual competency testing on paraprofessional staff.
• Act as a mentor when appropriate for other health care workers. Participate in quality assurance and performance improvement activities as assigned.
• Function as a field nurse supervisor as required.

Qualifications:

• R.N. diploma from an accredited NLN program with a minimum of an Associate’s Degree (BSN preferred). Valid and unencumbered RN license, certificate or registration or credentialing as required.
• One year of home health based experience is required.
• Minimum of three years of experience in medical surgical nursing preferably hospital based preferred. Two additional years of home-health based experience required for conducting applicable training classes.
• Strong assessment and organizational skills.
• Reliable means of transportation.
