Director of Social Work

Foltsbrook Center is actively seeking an experienced Director of Social Work for our Skilled Nursing Facility located in Herkimer, NY.

Previous management experience in an LTC setting required!

Job Type:

• Provide new resident orientation to the nursing home.

• Liaison for residents and families with nursing staff.

• Reviews and follows up on any concerns for the residents and staff with all incidents

• reports and investigations and report as appropriate and necessary. Schedules resident appointments and arranges for transportation.

• Assists with discharge planning as appropriate and necessary. Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience by providing a chance to develop new relationships with others, to foster personal growth and to continue to live as productive adults.

• Abides by the Resident's Bill of Rights.

• Conducts family meetings and nursing unit meetings.

• Develops and maintains a good working rapport with inter-departmental personnel as well as other departments within the facility to assure that the needs of residents are met.

• Serves on committees as assigned. Assumes duties as assigned by the Director of Social Services.

• Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents, staff and families.

• Assists Nurse Manager with resident related issues by offering managerial expertise in problem solving.

• Oversees care plans, MDS and quality improvement programs.

• Participates in survey, compliance and quality improvement processes to meet regulatory and licensing requirements. Abides by procedures and policies set forth by the facility and handbooks.

• Submits monthly goals reports as requested. Keeps supervisor fully informed of stats, progress and achievements. Attends and participates in continuing education programs and inservices as necessary.

• Serves as professional role model providing leadership in problem solving and all operations within the nursing unit on all shifts.

• Works closely with all staff to uphold and further the mission and values of The Diamond Nursing and Rehabilitation. Is highly visible on the nursing unit at various shifts and assists with care as needed.

• Collaborates with other Social Workers to share resources, knowledge, expertise, opportunities and achievements.

• Works with the Nurse Manager of the nursing unit to ensure residents receive the best possible service by being a strong advocate for the residents.

• Performs any other pertinent duties as assigned by the supervisor appropriate to the role and function of Social Worker.

• Must possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public.

• Must be able to cope with the mental and emotional stress of the position; is subject to frequent interruptions.

• Must be willing to work as a team player. Must be able to relay information concerning a resident's condition.

• Must be able to relate to and work with the ill, disabled, elderly and emotionally upset people within the facility.

REQUIREMENTS:

• Management Experience in LTC settings
• Must possess a bachelor’s degree in Social Work or Human Services field.
• Experience in Geriatrics preferred
• LSW – MSW
• Knowledge of applicable State & Federal guidelines
• Exceptional interpersonal & leadership skills
