The salary for this position is $27 per hour/Pay Grade I/HCP122.
The Social Service Provider is responsible for assessing clients’ needs and determining eligibility for Housing Opportunities for People with AIDS (HOPWA) services. The Social Service Provider develops an individualized service plan (ISP) for housing, emergency gap lodging, and Short-Term Rental, Mortgage, and Utility Assistance. The Social Service Provider assists with tracking monthly applications for HOPWA funds and is responsible for submitting monthly HOPWA reports.
Master's degree in a related field from an accredited college or university AND eligibility to be licensed AND Two years of related experience OR Bachelor's degree in a related field from an accredited college or university AND Four years of experience in social service delivery OR One year of experience at the lower level Social Worker, NonLicensed 2 (HCP121) or position equivalent
Experience working with People Living with HIV/AIDS is preferred
