JOB PURPOSE:
The Director of Home Care Services oversees the day-to-day clinical operations of all Best Choice Home Care programs in accordance with DOH, CMS, JCAHO, and other Federal, State, and local government agencies and the organization's goals. The Director of Home Care Services ensures that care is delivered in an efficient and cost-effective manner maximizing customer satisfaction.
JOB RESPONSIBILITIES:
- Oversees the day-to-day operations of the clinical components of the LHCSA.
- Interprets, reviews, implements, amends, and enforces policies and procedures of the organization by way of systems that will improve the overall operation and effectiveness of Best Choice programs
- Always maintains an appropriate level of clinical staffing for efficient operations.
- Prepares statistical and productivity reports as required by regulations, agency policies, contracts, and/or as requested by the Assistant Vice President (AVP) of Home Care.
- Monitors and ensures compliance with contractual, federal, state, and local rules and regulations concerning Licensed Home Care Agency services.
- Selects clients’ clinical records for monthly review to ensure compliance.
- Oversees the review of documentation on duty sheets, plans of care, infection control reports and logs, incident/accident/grievance reports and logs, and employee physical exams and assessments.
- Participates in the orientation of clinical staff.
- Oversees field supervision for paraprofessionals annually, including skills competency assessment, new hire visits, and visits for changes in equipment or patient status.
- Oversees in-service education programs for the clinical team and Home Health Aides.
- Provides field supervision of Clinical Care Managers annually as per DOH regulations. Takes referrals, admits, and provides follow-up nursing visits for private pay and contract case-managed patients in conjunction with the Clinical Managers.
- Creates and updates clinical forms for review by Assistant Vice President prior to implementation.
- Maintains a clinical on-call schedule.
- Participates in Quality Improvement and other assigned committees of Best Choice.
- Develops and implements performance improvement activities as needed, based on clinical issues, documentation findings, and/or new agency or industry Policies & Procedures.
- Prepares QI reports for Quality Improvement Committee on a quarterly basis.
- Performs at least weekly site visits to all branches of the agency.
- Communicates and collaborates with the Human Resources Supervisor and Director regarding health status and compliance issues related to Home Health Aides.
- Coordinates team meetings as needed to monitor the delivery of coordinated healthcare services to clients.
- Interfaces and communicates with DOH, CMS, JCAHO, and other Federal, State, and local government agencies on behalf of the organization and utilizes and implements a wide range of continuous quality improvement strategies
- Obtains and maintains current information and knowledge of the Home Care environment.
- Communicates with customers in a caring, helpful, considerate, and culturally sensitive manner.
- Suggests and implements customer-centered service improvements.
- Utilizes service recovery techniques to resolve issues quickly and to the customer’s satisfaction.
- Maintains positive “customer” relationships (“customer” is defined as co-workers, participants, visitors, vendors, and supervisors).
- Collaborates with consultants, contractors, referring physicians, community resources, government agencies, and advocacy groups.
- Works collaboratively with the Business Development team to identify suitable referrals for the agency.
- Conducts clinical outreach programs with Business Development to increase the potential client base for the agency
- Actively promote Best Choice Home Care programs to the community, major customers, government agencies, and organizations. Develops and maintains relationships with leaders of other healthcare organizations

Schedule: 8:30 AM - 5:30 PM
QUALIFICATIONS:
Education: A Baccalaureate Degree in nursing or other healthcare-related fields is required.
Experience: Possesses a minimum of 5 years of management experience in a Licensed Home Care Agency (LHCSA – CHHA) experience.
Other: This role will require bending, sitting, stooping, and standing. Most of the work of this role will take place in an office setting, with random field visits. Occasional evening, weekend, and holiday work may be required. Pay: $125,000.00 - $130,000.00 per year
