Stephenson Nursing Center (formerly known as Walnut Acres)

New leadership: This Skilled Nursing Facility (SNF) is owned and operated by Stephenson County, but now managed by Transitional Care Management. And we are expanding our team.

With 148 beds, we serve a diverse population of short-term rehab patients, long term care residents and memory care. We have:

What you will do in this role: You will work closely with our Marketing, Clinical and Administrative team to drive census. One moment you may be checking bed availability, the next checking a med price and the next is working through an insurance issue. All the while managing people, reports and data. This is a fast pace position and you need to have the ability to respond quickly, juggle multiple priorities and provide the ultimate customer service.

Admissions Director: Officially we title this job an Admissions Director, but in other centers it may be called an Intake Coordinator, Marketing Director or a Liaison.

What we need from you:

• 3+ years prior experience as an admissions coordinator/director in a Skilled Nursing Facility
• Associates degree or higher is preferred
• Experience with Matrix EMR software is helpful
• We follow the CMS & IDPH rules for the Covid-19 vaccine. You should be fully vaccinated or you will be required to submit an exemption. Details will be discussed during your interview.
