JOB SUMMARY

Under the direction and supervision of the Sr. Director, this position coordinates the case management for students providing exemplary customer service with external agencies and internal team members. This team member must have a comprehensive understanding of the IEP process, IEP development, Virginia State Department of Education Requirements, and federal laws surrounding special education.

All work is performed safely, effectively, to the highest ethical standards, and in accordance with regulatory and accrediting standards based upon the St. Joseph’s Villa’s mission, values, and guiding principles.

ESSENTIAL JOB FUNCTIONS

1. Maintains exemplary customer service and collaborative working relationships with all external agencies and internal team members while demonstrating appropriate professional behavior and dress at all times

2. Maintains case management records and ensures compliance with licensing and accreditation regulations. Compiles information to prepare and disseminate narrative, tabular, and statistical reports including but not limited to progress reports, monthly reports, report cards, Family Assessment Planning Team (FAPT) documentation, and placement agreements for assigned students. Maintains contact with designated family members or guardians, and school systems with a focus on customer service and collaborative practice.

3. Maintains and ensures compliance with all federal and state regulations in monitoring and developing cumulative files for all assigned students with file reviews that meets or exceeds agency documentation requirements. Regularly audits cumulative files to ensure compliance.

4. Schedules and communicates with internal and external stakeholders regarding all Individualized Education Plans (IEPs) and triennial assessments while ensuring compliance with all local, state, and federal guidelines.

5. Attends scheduled training programs for professional development that includes, at a minimum, trainings required by St. Joseph’s Villa and by regulatory and accrediting bodies.

6. Provides administrative support for SDCA, including answering phones, sending and receiving facsimiles, photocopying, maintaining time records, data input, and drafting correspondence.

7. Collection and review of initial enrollment paperwork for new students including following up with stakeholders to gain all necessary documentation.

8. Completes all required reports, documentations and data collection within required guidelines.

9. Monitors and maintains confidentiality for all students, families, files, and communication. Maintains record retention and historical filing

10. Seeks out opportunities for quality improvement individually and organizationally by submitting recommendations for continuous quality improvement to the Program Director

11. Maintains a safe and clean working environment while complying with SDCA’s policies and procedures

12. Monitors medication administration and consistently audits procedural fidelity.

13. Represents St. Joseph’s Villa at governmental hearings including courts and with the public school systems.

14. Performs other duties as assigned.

QUALIFICATIONS

BS/BA degree in Education, Psychology, Human Services, or related field. Extensive knowledge of local, state, and federal guidelines in education. Two years related experience. Proficient with Windows, Outlook, internet access, and MS Office including Word and Excel. Experience with proprietary software preferred. Valid driver’s license with excellent driving record.

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of ethical and legal guidelines in relationship to local, state, and federal education mandates. Knowledge of the educational system. Able to work effectively with all stakeholders providing exceptional customer service. Excellent verbal and written communication skills. Proficient in basic mathematics and elementary statistical techniques. Able to organize case load to maximize time utilization with no diminution in quality of service. Able to work effectively in collaborative, shared leadership environment. Able to be flexible and appropriately handle stressful situations. Able to appropriately use proprietary software. Able to be an effective role model.

FLSA STATUS: Non-exempt

Job Type: Full-time

Pay: $44,990.00 - $50,000.00 per year
