Outreach Efforts and Program Recruitment
- Identify community partnerships and serve as the liaison in the community for the Mobile Health Program (MHP).
- Manage social media platforms by creating and sharing posts.
- Create the monthly public-facing calendar and send out to community groups.
- Participate in health fairs and community events to promote the MHP and manage all supplies/equipment needed for these events.
Patient Support
- Attend prenatal patient clinics (1-2x per week) to orient new patients to the program, distribute prenatal bags, support patients in the application for Emergency AHCCSS, and explain self-pay fees and support in the Financial Assistance application for delivery at Banner.
- Attend (1-2x per week) Family Medicine Clinics to orient new patients to services and community resources.
- Link prenatal patients with delivery services at BUMC-T to maintain continuity with the patient care team.
- Advocate for self-pay patients during the Banner Billing process.
- Assist patients in postpartum navigation of family planning services and resources.
- Linking clients to services and community resources.
Administrative
- Support program administrative support as needed (answering phones, emails, attending meetings, conference attendance).
- Manage health education brochures and fliers for program.
- Involvement in marketing/branding decisions.
- Attend monthly staff meetings.
- Update and create database of community resources for patients.
- Collect data on outreach worker job functions (number of outreach events, number of referrals to community services, etc).
- Bachelor's degree or equivalent advanced learning attained through experience required.
- One (1) year of relevant work experience required.
- Must be fluent in English and Spanish
