The Community Wellness Nurse Charge Nurse provides care and service to Independent Living residents in the Wellness Center and may provide occasional coverage or emergency response within other departments.
Primary Responsibilities:
- Identify, assess and monitor changing resident health needs. Provides medical healthcare as described in physician orders. Coordinates with Assisted Living and Community Health Services Director when necessary to effect resident transfers from independent living.
- Responds to medical emergencies in Independent Living during Wellness Center hours.
- Establishes and maintain positive interpersonal relationships with residents, families, staff, vendors, and medical community.
- Reviews assistance alternatives with residents and their families/responsible parties including home care services.
- With the Director, develops, maintains, and periodically updates written policies and procedures that govern the day-to-day functions of the department.
- Oversees development of, and maintain process for, periodic updating of resident health & emergency response information in Independent Living.
- Directs operations of Wellness Center, including staffing, services offered, and adherence to operational and applicable State regulations.
- Maintains communication with physicians, families and appropriate staff regarding resident health status.
- Oversees and maintains current schedules of care and chargeable services. Submits itemized resident charges to the Finance Director monthly.
- Responsible for the coordination of clinical and non-clinical services for residents including the procurement of outside sources such as home health care services, hospice, rehabilitation services, psychological, etc. necessary to ensure that residents’ needs are met. Communicates and meets with outside consultants as necessary to assess and make recommendations for resident care needs.
- Supervises the Community Health Services staff. Conducts evaluations and performs disciplinary process as necessary.
- Regularly attends the morning meeting, Transition Care Planning, Utilization Review, Resident Health Care Conferences, Leadership, New Resident orientation and other committees or meetings as assigned.
- Serves as a member of the clinical leadership team.
Qualifications:
- Bachelor of Science in Nursing is required.
- Current license as a Registered Nurse in the state of Maine.
- Must be CPR certified or able to recertify prior to starting position
- Two years of management experience is preferred and five years of clinical experience as a licensed nurse is required.
- Knowledge of Independent Living is preferred and experience in senior care or aging services is required.
- Knowledge of application of current nursing techniques and procedures.
- Comprehensive knowledge of nursing service policies and procedures and standards or practice.
- Ability to work with a team and demonstrates a positive attitude and is a good role model for all staff.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Proficiency with Microsoft Office Suite and ability to utilize technology. Familiarity with MatrixCare or similar electronic medical records system.
- Ability to communicate with, understand, empathize with, work with and maintain professionalism with residents, families, visitors, peers, supervisors, and direct reports.
