The Primary Health Network is the largest Federally Qualified Healthcare Center (FQHC) in the state of Pennsylvania. We have been serving our communities for 36 years. We currently operate healthcare centers in 16 counties in the state of Pennsylvania and 1 in the state of Ohio. We offer primary care, behavioral health, specialty care (in select counties) and dental services across the state of Pennsylvania.

Position Overview

The Community Equity Coordinator works to improve the overall health and wellness of vulnerable populations within the Buhl Regional Health Foundation Footprint. The Community Equity Coordinator collaborates internally as well as with external partners to connect vulnerable populations to resources equitably; including Social Determinants of Health, COVID-19 education/intervention, and direct transportation. The position will be responsible to identify and engage social service agencies, local businesses and the general public in order to advance the program and organizational goals related to health equity and population health.

Location and Hours of Position

The general office hours of the position is 9:00am - 5:00pm. Hours may vary to include some evenings and weekends depending upon activities.

Education/Experience Requirements

• Bachelor’s degree in related field including nursing, social work, and/or public health required.
• 2 years outreach/healthcare experience preferred.
• Strong verbal/written communication and organizational skills.
• Computer literate.
• CPR Certified.
• Must have reliable transportation.
• Travel required – including, but not limited to, the defined PHN geographic service area(s).
• COVID Vaccine Required.

Duties

• Will be responsible in developing, supervising and implementing “whole-person health” by incorporating Social Determinants of Health concepts to promote health equity and improve community health outcomes.
• Will assist administration in planning, researching and organizing the Equity Team effort in outreaching to the communities PHN serves.
• Explores specific barriers to health care access and develop and implement plans to address those barriers community-wide and individually
• Coordinates interventions involving PHN departments, community groups, organizations, to support health equity and improved health outcomes.
• Work closely with Health Equity Advisory Committee and Community Impact Effort Committee to identify and reach disadvantaged populations and individuals in designated region(s).
• Develop and implement informational health related programs that will foster and grow community partnerships and connect potential patients to Primary Health Network services.
• Assist with the organizational and community response to COVID-19 mitigation to support equitable access to vaccines, testing, and therapeutics.
• Create and maintain key community contacts through networking, community events, etc.
  
• Coordinate proper outreach messaging with Equity Advisory Committee.
• Advocates for individuals and the immediate community.
• Develop and support Health and SDOH screening processes, data collection and management efforts to support program evaluation and outcomes reporting.
• Participates in evaluation and research related to health equity in designated areas.
• Other related duties as assigned.
